We are looking for an enthusiastic Care Coordinator to work across the Sutton OP CMHT based in North Cheam, which is one of the 4 teams within the Sutton OP services. It is a multi-disciplinary team comprised of Consultant Psychiatrists, Junior doctors, Team Manager, Nurses, OTs, Psychologists, and RSWs. The team specializes in assessing and treating adults aged 75 and over with a mental health condition.
Applicants need to be professionally qualified and committed to providing responsive and high-quality clinical services to mental health service users and their carers.
Main duties of the job
This role is for a Care Co-ordinator to work with patients with cognitive impairment and functional illnesses and requires extensive relevant experience of working with Older People within a range of settings. Applicants must have a keen interest in all mental health issues affecting Older People and up-to-date knowledge of treatments and protocols. The post holder will be able to work autonomously, exercise clear and coherent decision-making skills, and work flexibly to meet the needs of the wider service. The post sits alongside experienced clinicians of various disciplines, both within the CMHT as well as other sub-teams.
Applicants will need to have
* Extensive relevant experience of working with Older People in a range of settings, ideally with some community experience (not essential).
* A keen interest in all mental health issues affecting Older People and up-to-date knowledge of treatments and protocols.
* The ability to work autonomously and exercise clear and coherent decision-making skills.
* The ability to work flexibly to meet the needs of the wider service. We will consider a band 5 to band 6 development post for the right individual.

Job description
Job responsibilities
1. Facilitate, promote and maintain a consistently high standard of individualised client care, recognizing factors and events that are likely to affect the mental, emotional, and physical well-being of clients. Provide advice and support when required.
2. Identify risk and how risks are to be managed in this client group. Inform the Team Manager and other relevant staff where excessive risks are identified.
3. Act upon referrals allocated within the Community Mental Health Team within stipulated response times.
4. Carry out expert holistic assessment of service users and carers in the community setting.
5. Act as Care Co-ordinator and be responsible for ensuring that clients' needs and strengths are met through a coordinated approach and all clients have their care planned, implemented, and regularly reviewed/updated.
6. Maintain a high standard of practice through the use of a range of psychological approaches, within a health and psycho-social framework. Evaluate approaches and framework regularly through documented research and audits.
7. Carry out the drug administration in accordance with the Trust Policy and the NMC Standards for Administration of Drugs, giving advice, information, and support to staff and clients/carers.
8. Maintain independence and assist clients to perform daily activities for themselves as appropriate.
9. Work collaboratively with the multi-disciplinary team and other agencies (e.g., GPs, District Nurses, carers). Ensure that an integrated program of care is provided in line with Risk Assessment procedures.
10. Develop, educate, and establish purposeful individual and group programs of care, monitoring and evaluating individual clients' needs/developments.
11. Act as a specialist resource for information, training, and advice to clients/carers and other healthcare professionals.
12. Organize and participate in the admission, transfer, and discharge of clients in accordance with the Trust policies.
13. Implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies.
14. Act as a link person for the team with GP practices.
15. Build up a healthy alliance of community networks (e.g., Health promotion, Social Services, Housing, Police, and other agencies).
Person Specification
Experience
Essential
* Minimum of 3 years' experience at Band '5' level, preferably with some in a community setting, ideally within older people's mental health services.
* Good time/caseload management, CPA, Clinical Supervision.
* Significant relevant clinical experience.
* Experience as a preceptor/mentor.
Desirable
* Experience of taking responsibility for a ward/clinical unit.
* Experience of engaging/supporting clinical research projects.
* Experience of supporting practice development in clinical areas.
Training and Qualification
Essential
* Registered Adult Nurse and/or Registered Mental Health Nurse and/or Registered Learning Disabilities Nurse.
* Evidence of ongoing personal development/education in line with PREP.
Desirable
* Master's degree.
Knowledge & Skills
Essential
* Sound knowledge of clinical and managerial issues relating to Mental Health and older people's services.
* Ability to work well within the multidisciplinary team.
* Ability to manage difficult situations and cope with stress and change.
* Knowledge of PREP, Clinical Audit.
Desirable
* Understanding of evidence-based practice.
* Understanding of the mental health needs of different cultural and ethnic groups.
* Aware of NSF (Older People).
