Job overview
We are looking for an excellent Community Practitioner who has patient care at the heart of their practice. We have a strong teaching/development focus and are delivering ongoing training in psychological interventions to ensure our staff feel equipped to meet the needs of the role.
Main duties of the job
1. Be responsible for the assessment and management of casework of service users who have substantial and complex needs.
2. Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies, and the public.
3. Develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plans as necessary.
4. Consider Carer needs and ensure Carer and family involvement in care planning where appropriate.
5. Deliver evidence-based clinical and psychologically based interventions.
6. Work as a member of the Multi-disciplinary Team ensuring high-quality individualized care is delivered.
7. Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning.
8. Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.
9. Provide compassionate care based on empathy, kindness, respect, and dignity.
10. Incorporate a client-centered philosophy into nursing practice and client-centered collaborative care.
Person specification
Knowledge and Experience
* 12 to 24 months band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* Understanding of difficult to manage/challenging behaviours.
Education and qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Evidence of post-registration training in clinical psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Experience of working in a community team.
* Qualified Non-medical prescriber.
* Post-registration qualifications in psychological interventions.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Demonstrate an understanding of effective and appropriate treatment options including a range of experience-based clinical approaches.
* Clinical leadership.
