Job summary
Oakwood Lane Medical Practice has a great opportunity for a GP to join our team for 4-6 sessions per week, to include Thursday and Fridays.

8. Extended Access support - Healthy Minds practitioner,Virtual MSK & meds management, Saturday Hub on site with GP / Nurse/HCAappointments run by Leeds GP Confederation.
9. Fortnightly clinical meetings, daily coffeecatch up meetings

Main duties of the job
Conducting face to face and virtual appointments
Processing PATCHS clinical queries and telephone triage
Participating in the on call rota and the triage lead in the Appointment Hub
Admin - arising directly from your caseload (referrals, investigations, results) and indirectly (reports, medicals, etc)
Visits when required

Job responsibilities
10. Undertake a variety of duties, including surgery consultations, telephone consultations, video consultations and queries
11. Share of oncall days and lead for Appointment Hub triage.
12. Visiting patients at home when required
13. Checking and signing repeat prescriptions when required
14. Dealing with queries, paperwork and correspondence in a timely fashion
15. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
16. Assessing the health care needs of patients with undifferentiated and undiagnosed problems
17. Screening patients for disease risk factors and early signs of illness
18. Developing care plans for health in consultation with patients and in line with current practice disease management protocols
19. Providing counselling and health education
20. Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
21. Recording clear and contemporaneous consultation notes to agreed standards
22. Collecting data for audit purposes
23. Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
24. Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
25. In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
