Job Title: Nurse Case Manager – Paediatric Complex Care at Home
Salary: £45,000 - £50,000 per annum + mileage
Hours: Full-time, 37.5 hours per week, Monday to Friday
Location: Hertfordshire Patch - covering Watford, Borehamwood, Hemel Hempstead, St Albans & Chalfont St Peters areas (Remote & Field-based)

About the Role:
Compass Associates are proud to be working with a leading provider of complex care at home, recruiting for a Nurse Case Manager to oversee a caseload of 9 paediatric clients across the Watford, Borehamwood, Hemel Hempstead, St Albans, and Chalfont areas. The clients live with complex conditions such as Cerebral Palsy, Brain Injury, and Neuromuscular Disorders. As the Nurse Case Manager you’ll ensure high-quality, person-centred care is delivered, supporting care teams, families, and working collaboratively with multi-disciplinary professionals.
Key Responsibilities:
* Case Management: Lead and manage paediatric complex care packages across the Hertfordshire patch.
* Clinical Oversight: Maintain and review care plans, risk assessments, and ensure staff are clinically competent.
* Training & Support: Provide specialist training in areas such as tracheostomy care, ventilator management, and PEG feeding.
* Team Collaboration: Work closely with internal care coordinators and external agencies to ensure a smooth care experience.
* Quality & Compliance: Conduct regular reviews, assessments, and supervisions to uphold outstanding care standards.
About You:
* Registered Nurse (RGN / RSCN / RCN) with an active NMC pin
* 3+ years of paediatric complex care experience in hospital or community settings
* Confident in tracheostomy care, ventilation, and PEG feed
* Background in community nursing or home-based care
* A driver with access to a car
