This position requires an autonomous specialist lead in developing, co-ordinating, evaluating and managing the Lymphoedema and Tissue Viability Service. The post holder will use advanced clinical expertise combined with managerial and leadership responsibilities to improve patient care, ensure efficient operations and promote positive teamwork. This role will also promote clinical excellence through the provision of consistent specialised care, skilled communication, evidence based practice and continuous professional development of staff. The TVN will work collaboratively to implement audit processes and systems within Pioneer for the purpose of promoting clinically effective and continuously improving care. KEY RESPONSIBILITIES AND ACCOUNTABILITIES Perform complex holistic wound assessments following referral from healthcare professionals. Implement care with patients and carers using diagnostic tests or equipment where appropriate e.g. Doppler and full vascular assessments. Be able to demonstrate/obtain clinical skills such as conducting full vascular assessments including differential diagnosis. To support triaging of clinical assessments and care planning via telemedicine for patients as appropriate. Respect people's dignity, wishes and beliefs; involve them in shared decision making and obtain their consent before undertaking procedures. Promote a patient focused multidisciplinary approach to care in collaboration with carers, health care professionals and other agencies. To work according to the NMC Code of Professional Conduct and relevant professional guidelines as the Lead clinician for a defined group of patients.
