Job summary
We are looking for an enthusiastic Practice Nurse to join our friendly, forward-thinking and innovative team. You will be working alongside 2 other experienced practice nurses, providing the whole range of practice nursing duties. We encourage and support learning and professional development and have access to training from our CCG.
Main duties of the job
1. Provide direct patient care, including conducting assessments, administering vacines and performing treatments
2. Assist healthcare providers during examinations and procedures
3. Monitor patients' vital signs and record medical information accurately
4. Educate patients on health management and provide guidance on preventive care
5. Collaborate with the healthcare team to develop and implement patient care plans
6. Maintain accurate and up-to-date medical records
7. Ensure compliance with infection control protocols and maintain a clean and safe environment
8. Adhere to professional standards, ethical guidelines, and legal requirements

Primary responsibilities:
The following are the core responsibilities of the practice nurse. There may be, onoccasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels and current competency:
23. To implement and evaluate individual treatment plans for chronic disease patients delivering patient care as necessary, referring patients to secondary/specialist care as required
24. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health.
25. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
26. To provide wound care to patients
27. To provide travel medicine services
28. To request pathology services as necessary
29. To maintain accurate clinical records in conjunction with extant legislation
30. To ensure read codes are used effectively
31. To maintain chronic disease registers
32. To develop, implement and embed well woman clinics
33. To chaperone patients where necessary
34. To support the team in dealing with clinical emergencies
35. To recognise, assess and refer patients presenting with mental health needs
36. To implement vaccination programmes for adults and children
37. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
38. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
39. To support the clinical team with all safeguarding matters in accordance with local and national policies
40. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
41. To deliver opportunistic health promotion where appropriate

42. Monitor and ensure the safe storage, rotation and disposal of medicaments.
43. Support members of the nursing/clinical team.

46. Continually review clinical practices, responding to national policies and initiatives where appropriate.
47. Participate in the review of significant and near-miss events applying a structured approach, root cause analysis (RCA).
