Job summary The Sentinel Stroke National Audit Programme (SSNAP) aims to improve the quality of stroke care by measuring both the structure and processes of stroke care against evidence-based standards. These standards are informed by the National Clinical Guideline for Stroke, and national and local benchmarks. SSNAP is an audit that measures the quality of care that stroke patients receive throughout the whole care pathway up to 6 months post admission in England, Wales and Northern Ireland. SSNAP offers clinicians, policy makers and the public a reliable and comprehensive source of data to inform quality improvements. This role will have responsibility for capturing and monitoring SSNAP audit data across the Health Board as well as supporting Directorate Manager/Deputy Directorate Manager with administration of Stroke related meetings. The post holder will work closely with Stroke medical, nursing and allied professional teams, will attend multidisciplinary team meetings to ensure the data is validated and captured arcuately. There is an opportunity to enhance current systems and processes within the team and this role will be instrumental in developing this with senior management support. The successful candidate will have excellent analytical skills, a keen eye for detail and a good understanding of Microsoft Office packages. The role will be based at the Grange University Hospital with some travel required across other hospital sites. Main duties of the job Responsible for developing and maintaining effective communication and working relationships across the Hyper Acute Stroke Unit (HASU) in GUH and the Stroke rehabilitation wards in Ysbyty Ystrad Fawr (YYF). Liaise with consultants, nursing and allied professional teams to ensure data is captured in an accurate and timely manner. To handle information with high levels of sensitivity and confidentiality. Provide administrative support during Stroke Directorate meetings, record minutes and actions.
