Job responsibilities
* Be primarily responsible for the rapid assessment and treatment of patients who have urgent and/or complex physical, mental health, and social care needs.
* Provide health care advice and education to team members, statutory, voluntary, and independent sector providers.
* Be responsible for health promotion, teaching, and training of support workers, pre-registration students, patients, relatives, and carers.
* Ensure up-to-date written and electronic records and activity data are maintained in accordance with professional and trust standards.
* Prioritize caseload, with professional demands, considering the needs of other members of the team.
Person Specification
Experience
* Community experience.
* Experience in discharge planning of patients from hospitals.
* Experience of caseload management and prioritization.
* In addition to experience, all qualified roles will be required to provide evidence of:
* Registration with the relevant Registration Body.
Qualifications
* Current registration with HCPC.
