About the role:
The Home Support service looks at supporting those living in the community to remain living in their homes for as long as possible, leading the life they want to whilst maintaining independence.

* Basic admin / IT skills.

What you will do:
* Complete regular welfare calls both via telephone and in person to complete well-being checks on members of the service to ensure they are safe and well. Listening to members and understanding what is important to them.
* Signpost to relevant agencies / professionals (OT’s / DN’s etc) to support individuals' needs, identifying changes or deterioration in health/ability and what support could make a positive difference.
* Support in emergencies, reducing the need for emergency services – this requires working a rota pattern out of hours responding to call outs.
* Complete light admin duties reflecting input provided when supporting individuals within the community.
* Work from home and be willing to work nights, weekends, and Bank Holidays when on the rota. A current driving licence and use of a car is essential.
* Provide personal care to users of the service in coordination with Radnorshire Home Care Service.

Service: Adult Social Care
Salary: Grade 5 Point 7 to Point 9 £25,584 to £26,409 per annum pro rata £13.26 to £13.68 per hour
