The Home Support service looks at supporting those living in the community to remain living in their homes for as long as possible, leading the life they want to whilst maintaining independence.

What you will do:
• Complete regular welfare calls both via telephone and in person to complete well being checks on members of the service to ensure they are safe and well. Listening to members and understanding what is important to them.
• Signpost to relevant agencies / professionals (OT’s / DN’s etc) to support to ensure the needs of the individuals are met therefore being able to identify a change or deterioration in one’s health / ability and identify what support would/could make a positive difference.
• Supporting in the event of an emergency, where possible reducing the need for emergency services – this requires working a rota pattern out of hours responding to out of hours call outs.
• Complete light admin duties to reflect input provided when supporting individuals within the community.
• The post holder will be required to work from home and must be willing and able to work at night, weekends and on Bank Holidays when on the rota to do so. The post holder must also hold a current driving licence and the use of a car will be essential to the post.
• To provide personal care to users of the service in co-ordination with Radnorshire Home Care Service.
