We currently have a vacancy within our service and are looking for motivated and enthusiastic Learning Disability Nurses to join our team.
The role involves assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (revised 2022), planning and commissioning packages of care (including Personal Health Budgets), supporting individuals, their families, and health professionals through the Continuing Healthcare process (including transition from child to adult services) and reviewing eligibility/packages of care.

Main Duties of the Job:
We need a nurse practitioner who is experienced working with people who have learning disabilities - this is an essential characteristic.
NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital but who have complex ongoing healthcare needs.
Devon Partnership NHS Trust is responsible for the decision-making and commissioning processes, working closely with the Local Authorities on behalf of Devon ICB.
Successful candidates will have excellent communication and assessment skills; be resilient, able to work autonomously but also as part of a team. This is a community-based post and although the role is based in Mid Devon, post holders will be expected to travel across the whole of Devon, and occasionally out of the county.

Job Responsibilities:
To actively engage with a wide range of stakeholders including staff, service users, their carers, and other statutory and voluntary sector agencies to ensure people are fully briefed and understand commissioning and care management processes in relation to Continuing Healthcare, Funded Nursing Care, health and social care funded packages of care and section 117 aftercare.
* Impart complex and sensitive information to:
* Service users and families/carers regarding assessment, care planning, treatment, and review.
* Multi-disciplinary teams to ensure the provision of consistent well-coordinated care.
* Primary care teams regarding client care.
* External agencies involved in the provision of care e.g.: voluntary and private sector, police, social services, housing, employment etc.
* Safeguarding, DOLS teams, Clinical Commissioning Groups, and Devon County Council regarding concerns arising from commissioned packages of care.
Person Specification:
Qualifications:
* Registered Nurse - with Learning Disability experience.
* Postgraduate qualification relevant to specialism.
Knowledge and Skills:
* Sound understanding of the Continuing Healthcare Framework, including Funded Nursing Care and Section 117 aftercare.
* Knowledge and understanding of Mental Health and Mental Capacity Acts.
* Discretion and confidentiality.
* Negotiation and motivational skills in hostile and complex situations.
* Ability to work alone and as part of a team.
* Excellent clinical assessment skills.
* Skilled at providing clinical, professional and management supervision and engaging and liaising with colleagues.
* Reflective and responsive to feedback.
* Clarity of thought and creative approach to problem solving.
Experience:
* Demonstrates experience of effective multidisciplinary and multi-agency work.
* Experience of working with people with a Learning Disability with complex physical health, challenging behaviour, and/or mental health needs.
