Advanced Nurse Practitioner/Advanced Clinical Practitioner
As an Advanced Clinical Practitioner you will be an experienced nurse/paramedic who, acting within your professional boundaries, will provide care for patients registered at any of the care homes, patients' homes and GP practices in the Durham Dales area from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
You will be expected to work across all of the services provided by DDHF.
Main duties of the job
As an Advanced Clinical Practitioner you will be working across the following services:
1. home and care home visits
2. long term conditions reviews
3. palliative care
4. telephone triage
5. Extended primary care access services (evenings, weekends, bank holidays)
6. Clinical support within practice
7. Enhanced health in care homes
About us
12 GP practices across Durham Dales got together, looking for a solution to help them provide the care patients needed, share resources and ideas, & be prepared for the challenges the NHS would face in the future.

Job responsibilities
1. Assess patients presenting with acute illness or for review of chronic disease conditions
2. Undertake home visits for acute illness or chronic disease management
3. Diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
4. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
5. Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
6. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
7. Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug-based treatment methods into a management plan
8. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies
9. Work with patients and their carers in order to support compliance with and adherence to prescribed treatments
10. Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
11. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
12. Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
13. Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health
14. Undertake minor surgery as appropriate to competence
15. Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
16. Communicate with and support patients who are receiving bad news
17. Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating
18. Maintain effective communication within DDHF and with external stakeholders
19. Attend and contribute to team meetings
20. Undertake mentorship for more junior staff, assessing competence against set standards
Person Specification
Qualifications
* Registered first level nurse
* MSc in Advanced Clinical Practitioner or equivalent OR must be willing to work towards

Experience
* Minimum 2 years post registration experience
* At least 2 years recent primary and community nursing experience
* Nurse-led management of minor illness, minor ailments and injuries
* Nurse-led triage including telephone triage
* Partake in clinical audit
* Project management
* Working with community development initiatives
* Health-needs assessment
* Home visit assessment of patients
* Community nursing specialist qualification
* Leadership in quality initiatives such as clinical benchmarking
* Clinical supervision training and experience
Other
* Full UK driving Licence
* Willingness to travel throughout the Durham Dales area
* Prepared to work out of hours on a staff rota
* Highly motivated
