The Individual Commissioning Team is responsible for assessing, commissioning, and case management for individuals with complex health needs.
As a Duty Clinician, you will provide the initial point of contact for the individual commissioning team. You will meet their families and be required to produce a thorough and accurate review of their Fast-Track pathway (face to face), which may then require completion of a Decision Support Tool. You will have to be able to make a holistic positive difference to the lives and experiences of those you assess. You will need to be fully competent in the National Framework for CHC, including Fast-Track and FNC, and operate within local and national requirements.
The role will support the Service Manager with ensuring that statutory responsibilities are met. This may include support with the Duty Triage process if required.

Service operating hours are Monday to Friday 09:00 – 17:00.
You must possess a current NMC (Adult, Mental Health or LD posts available), England professional registration and have the ability to travel within the locality.
You will be a competent professional with clinical experience within your field of practice and have a full understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care, Who Pays guidance, and relevant sections of the Mental Health Act and commissioning and contracting principles.
Ideally, you will be an experienced CHC assessor; however, if you are an experienced Nurse, England registered professional and do not have CHC experience, we would still like to hear from you.
Key Responsibilities:
* The Duty Clinician will co-ordinate all hospital discharge referrals, CHC fast track, and all urgent matters where the allocated case manager is unavailable. They will work as a daily point of contact for all matters requiring an urgent response.
* The post holder will have a flexible approach to care and will work across inter-agency boundaries, maintaining good communication and liaison skills with all professionals and members of the public.
* The post holder will lead the commissioning and management of packages of care for people in a variety of care and support settings.
