The job holder will carry out any other duties as may reasonably be required by their line manager The job description and person specification may be reviewed on an ongoing basis in accordance with the changing needs of the department and the organisation Primary Care Supporting the development of robust estate management processes to secure a rationalised, efficient, and quality estate and enabling the transformation of services through partnership working and identifying opportunities to develop the estate as a health and social care system Being the estates point of contact for those within their place based portfolio, (Bassetlaw, Nottingham City/South Notts/Mid Notts), and developing strong relationships with practices, PCNs and other stakeholders including partners within the ICB Reviewing PCN estates strategies and primary care premises plans ensuring that a clear picture of the utilisation of the existing estate is developed to enable space to be maximized in terms of occupancy and efficiency Working closely with place-based partnership colleagues and practices to ensure engagement and understanding of the Primary Care Network Estates Toolkit and supporting them throughout the journey Project managing individual schemes for practices in their portfolio from initial discussions and supporting PIDs through to gaining approval through via the correct governance channels and overseeing delivery. This will involve understanding practice specific issues, such as GP debt, practice history and revenue consequences whilst also building stakeholder relationships Becoming a competent user of the SHAPE tool. Enabling the post holder to understand their place as part of the ICB and provide the stats for informed decision making Ensuring the primary care data gathering and 6 facet surveys are complete and relevant databases up to date for their relevant place Ensuring that estates plans are in the best position to apply for any opportunistic funding including commissioner capital, Section 106 etc including planning the timing and production of PIDs, outline business cases and full business cases. Working with district councils to request S106/CIL funding from new housing developments to ensure that new residents have adequate primary care services. This involves working with the practices in the local area to increase their available capacity with any funding secured Managing and monitoring the Business-as-Usual Commissioner Capital for primary care for their portfolio of estates, from initial discussions and supporting PIDs to evaluating and ensuring due diligence and robust processes through to NHS England approval Corporate Operational management of current and future corporate headquarters consolidation plans. The person who researches and plans the functionality on the estates and can confidently support the decisions made Working closely with the Head of Estates to ensure smooth mobilisation of the agile working model from an estates perspective, ensuring the bases can comfortably accommodate staff and have all the correct services in place System Managing ad hoc system wide project and research requests, applying own initiative, and reporting information along with suggested ideas/resolutions using a variety of sources and cooperation from various stakeholders Operational estates management within system level programmes such as the national vaccination Programme Deputising for the Head of Estates when required
