Job summary We have an exciting opportunity available for a Clinical Pharmacist to join our small but ever-growing GP Practice in Yate. As an integral part of a multi-disciplinary team, and with support of a GP Partner clinical mentor, in this varied role you will be responsible for providing structured medication reviews for patients, supporting long-term condition management, as well as project work such as CQC compliance, community collaboration and the overall management of prescribing systems. This is a part time role, working 17 hours per week consisting of a mix of remote working and patient facing work in the practice. Main duties of the job Responsibilities include but are not limited to: a. Conduct structured medication reviews for patients with polypharmacy, long-term conditions, and complex prescribing needs. Consultations may take place in person or virtually. b. Be a prescriber, working alongside a GP for mentorship and support c. Provide expert pharmaceutical advice to patients and the wider practice team, addressing medication queries and ensuring safe and effective prescribing. d. Manage repeat prescribing systems, supporting medicine optimisation and reducing waste. e. Process hospital discharge summaries and outpatient letters, ensuring seamless medicines reconciliation. f. Subject to training and experience, support long-term condition management, working within clinical guidelines for conditions such as diabetes, hypertension, COPD, and cardiovascular disease. g. Work collaboratively with community pharmacists, hospital and ICB teams, nursing homes and PCN colleagues to improve medication safety and continuity of care. h. Support and deliver quality improvement initiatives, including clinical audits and compliance with prescribing best practices. i. Participate in Care Quality Commission (CQC) compliance and ensure adherence to local and national policies on medicines management. j. Provide training and education to the practice team on prescribing updates, guidelines, and medicines safety alerts.
