Main duties of the job
* Assessment of patients on reablement unit including mobility, functional assessments, and cognitive assessments.
* Assessing for and ordering appropriate equipment.
* Delegation of appropriate work to Therapy Practitioners.
* Multidisciplinary working with physiotherapy and nursing staff.
* Discharge planning including liaising with relatives and external agencies.
* Completing timely referrals to appropriate teams to facilitate discharges.

Person Specification
Qualifications
* Current relevant professional registration (HCPC Registration).
* Minimum 2 years post-qualification experience (Occupational Therapy Degree/Physiotherapy Degree).
Experience
* Enhanced DBS.
* Experience of working within the NHS.
