This is a great opportunity for a highly motivated, dynamic and organised Band 4 practitioner to join the Adult Inpatient Division, Bed Management & Adult Inpatient Social Work Team. The team work closely with Adult Acute, PICU and Rehabilitation wards to help facilitate safe and effective admission and discharge to and from inpatient settings. Within this fast paced demanding, but highly rewarding role, you can expect to gain a wide range of experience including knowledge and understanding around treatment pathways, available resources for service users leaving hospital environments and develop close working relationships with all support services across Lincolnshire. This is a role with considerable responsibility; you'll be working alongside senior clinicians to ensure the best possible outcomes for all service users under the care of Adult inpatient Services., The post holder will be responsible for the implementation of care plans and support to services users experiencing severe mental illness and/or mental health problems across the inpatient wards. The post holder will work under the supervision of a team leader and joint work with social work and inpatient colleagues. Main requirements are to:
* Contribute positively to multi-disciplinary and multi-agency team workSupport people in Adult Acute, PICU and Rehabilitation Inpatient services aged between 18 and 65 who have severe and enduring mental illness
* Work with individuals to develop support plans, set outcomes and enable people to achieve those outcomes.
* Support individuals through the recovery process, to achieve the best quality of life possible and to be able to live as independently as possible.
* Identify and support with overcoming barriers to discharge
