An exciting opportunity has arisen for 2 x Band 6 Nurses fixed term for 12 months to work as a Link Worker with our Psychiatric Liaison Team.
The successful candidates will be the responsible Link Workers for the follow-up, support, and development of an informed plan of care (utilising Dialogue +) for our Service Users who are under the care of the Community Mental Health Teams and who have accessed A&E Services.
Main duties of the job
The role will be to do preventative work to reduce A&E attendance and to support with contingency plans, signposting, education around what other Services are available and can be accessed as an alternative to A&E, and build better links with their CMHTs.
Detailed job description and main responsibilities
The post holder:
* Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary.
* Will manage a defined caseload and provide supervision to others as appropriate.
* Will be responsible for the development of goal-based care plans utilising Dialog+ and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
* Will be responsible for ensuring effective communication pathways exist.
* Will be responsible for ensuring the promotion of equality, diversity, and rights in accordance with good practice and legislation.
* Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence-based care as appropriate.
Person specification
Essential criteria
* Registered Mental Health Nurse (part 3 or 13)
* Significant post-registration experience in mental health
* Comprehensive assessment skills
Desirable criteria
* Experience of working in community mental health services
* Student mentoring qualification
