ThisisaTrainee(Apprentice)role,duringwhichthepostholderwillbesupportedtoworktowardssuccessfulcompletionof:

Asanapprenticeshipthisroleissupportedbyanaccompanyingassessmentandskillsdevelopmentprogramme,duringwhichthe postholderwillgaintheskills andknowledgerelevantforthisrole.
Learning will be a mix of in the workplace, formal on and off the job training, and the opportunity topractisenewskillinarealworkenvironment.OnsuccessfulcompletionoftheirApprenticeship,theywillhavetherightknowledge,skillsandbehaviours they willneedfortheirchosenrole.

Key Duties
• Toliaisewithmortuarystaffandwardstaffonadailybasistoidentifydeathswhich haveoccurredinthehospital.
· Toliaisewithwardstaffandmedicalstafffor thetimely andaccuratecompletionofmedicalcertificatesofdeath.
· Tofacilitatethepreparationofcremationformsandliaisewithfuneraldirectors,doctorsandmortuarystaff.
· To verifyandprocessalldocumentsrelatingtotheregistrationof deathsforfamilies.
· ToliaisewithCoronersOfficersregardingreferralsofhospitaldeathsandrelative’senquiries.
· ToliaisewithGPSurgeriesinformingthemofthedateofdeathandcauseofdeathinatimelymanner.
· Beabletoworkaspartofateam,promoteeffectiveflowsinthedepartmenttocoverleaveandensureofficerunssmoothly.
· Workwithmanagerstoreviewworkingpractices,waysofworkingandfindsolutiontoproblems/updateprocedures.
· Liaisewithappropriatestakeholders/departmentsinarranginghospitalfunerals.
· LiaisewiththeGeneralRegistrar’sOfficeconcerninganyqueriesoveracceptablecausesofdeath.
· Liaise with Coroners Liaison Officer to obtain medical certificates of death where possible for therelatives of patients that have been recently discharged from hospital where the G.P. has beenunabletoissue,inordertosavetherelativesfromthedistressofthedeceasedhavingtogothroughapostmortem.
· Tosupply the Notification ofDeath report to other hospital departments that use stand-alonerecordingsystems.
· When circumstances require, contacting HM Coroner’s Offices concerning any reportable deathsgivinganyassistancethatmayberequired.
· TosupporttheBereavementOfficersingeneralofficeduties.
· Toassistintheproduction,maintenanceandimplementationofSOP’sandimplementationofnewprocesses.
· Participateinthenewdoctors’inductiondayinformingthemoftherequirementsoftheBereavementService
· Assist with theinvestigation andcompilation ofresponses tocomplaints, helping to ensure this isdonewithintheoptimumdeadlinessetbytheTrust.
· Recordingofdeathsandallappropriateinformationinthedeathregister.
· Updatethedeceasedpatient’snotesandarrangeforthemtobesentto the relevantdepartment.
· Compilemonthly/annualstatisticsforinternalandexternaluse.
· Make full use of the computer systems and packages available.Ensure all information is updatedregularly and take responsibility for reporting any faults, incorrect patient data and breaches insecurity.
· Collate audit data for Clinical Pathology Accreditation and Benchmarking, leading for Qualsys ondocumentationcontrolforDepartmentalManager.Undertakeauditof ownworkwhennecessary.
· Establish,maintainanddevelopprofessionalworkingrelationswithcolleagues.
· Make full use of the computer systems and packages available.Ensure all information is updatedregularly and take responsibility for reporting any faults, incorrect patient data and breaches insecurity.
· Effectiveverbalandwrittencommunicationshouldbemaintainedatalltimesbyextractingthecorrectandrelevantinformation todeal successfully withqueries/requests from GP’s,staff, patients,relatives and other agencies. Show empathy when dealing with calls of a distressing nature. Impartconfidentialinformationwheninstructed
· BeawareandresponsivetochangeswithintheTrustandadoptaflexibleandproactiveapproachtowork.
· Generalofficedutiesincludingdealingwithincomingandoutgoingmail,e-mail,photocopier.Participateinhousekeepingoftheofficeenvironment.
· Beabletoworkaspartofateam,promoteeffectiveflowsinthedepartmenttocoverleavetoensureofficeruns smoothly.Guidance/mentoringtonewclerical/medical staff.
· Anyotherdutiesasdeemednecessaryfor theeffectiveserviceforthedepartment.
· Undertakeauditsasrequired.
· Assistwithresearch/datacollectionasrequired.
· ObtaininformationrequestedbyCellularPathologyServiceLeadManager/ASCManagerasandwhenrequired.
· Workwithmanagertoreviewworkingpractices,waysofworkingandfindsolutiontoproblems.
· ToworkinaflexiblemannerinaccordancewithTrustPolicy.
· AnyotherdutiesrequiredbyASCManager.
Teaching and Training responsibilities
To provide training in all aspects of the role as necessary to less experienced members of staff.Attend appropriatetrainingand educationsessions attherequestof themanager.
Performance
The performance of the post holder will be continually assessed for professional competence by meansof an annual appraisal.Key elements of this process will be used to establish evidence of continuingprofessionaldevelopmentandwillcontributetothecorporateobjectivesofthedepartmentandtheTrust.
