The post holder will work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
Main duties of the job
The position is Part-Time, 30 hours per week. The role is structured with an organized workflow, with set appointment numbers, and the post holder should expect a mix of same-day and pre-booked diabetic review encounters.

Job Title: Diabetic Specialist Nurse
Main Purpose of Post: Triage, Consultations, and Home visiting. The role involves telephone triage, home visits, and interactions with the care homes linked with the surgery. The focus of care evolves around our diabetic patients.
Reporting to: Advanced Nurse Practitioner Manager
Interface with: Patients, Doctors, Nurses, Practice Teams, Primary Care Network Partners, ICB, Community Teams.
Location: Post holder will be required to work at The Thorpewood Surgery or Dussindale Branch Surgery as required.

Main duties and responsibilities of the job:
1. A point of contact within the practice for patients presenting with undifferentiated undiagnosed problems that require telephone triage, telephone resolution, face-to-face consultation, online consultation response, and home visiting.
2. Assessment, diagnosis, treatment, prescribing, referral, and discharge as appropriate and deliver key elements of Surgery initiatives.
3. Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
4. Instigate invasive and non-invasive diagnostic tests or investigations, interpretation of findings, and reports.
5. Directly admit patients to secondary care outlets in medical need.
6. Refer patients to other medical specialties for assessment where indicated, including fast track referrals.
7. Take responsibility for making autonomous decisions in relation to patient care.
8. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
9. Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs.
10. Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs.
11. Collect data and participate in regular Practice audit and research projects.
12. Work effectively with individuals in other agencies to meet patients and carers' needs.
13. Recognize and work within own competence and professional code of conduct as regulated by the NMC.
14. Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures on the Surgery clinical system.
15. Deliver care according to local and national guidelines and evidence-based care.
16. Provide expert Diabetes and nursing advice to others working within the team.
17. Maintain own 3 yearly registration with the NMC.
18. Maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.
19. Work with the clinical and management team to develop new services for patients.
20. Contribute to practice incentive schemes, disease management registers, and Surgery targets set locally and nationally for patient care.
Special Requirements of the Job:
An understanding, acceptance, and adherence to the need for strict confidentiality.
Competence in the use of IT systems, i.e. coding work as completed, with the ability to learn new software.
