An exciting job opportunity has become available to join the Macmillan Specialist Palliative Care Team, areas of recruitment likely to be Lincoln, Spalding and Grantham, specific area to be confirmed. Travel to various bases is required as part of the role.
The post holder will be required to work as part of the team to provide specialist advice and support on symptom management to patients and their families, registered to a Lincolnshire GP, to ensure delivery of safe, person centred and effective palliative care.
The post holder will be required to undertake shifts on the Single Point of Access Line, to provide remote advice to patients and their families for urgent palliative care enquires. The line operates 0800-2000, 7 days a week including bank holidays.
Applicants must be NMC registered Nurses (adult) with experience in palliative and end of life care.
Please note this position will not commence before May 2025
• You will work as an accountable and autonomous practitioner alongside the community Macmillan Clinical Nurse Specialists,
• You will work in a shared caseload approach, with supervision from the band 7 Macmillan CNS.
• Triage referrals for planned and unplanned care.
• You will provide specialist palliative care advice and support to patients and those important to them, who have a palliative diagnosis with unstable needs.
• Provide specialist support and education on all aspects of palliative care to members of the multi-professional team in a variety of settings.
• Assess requirements for bereavement care and support for those carers and family members known to the service.

• Manages own workload, with support from band 7 clinicians
• Undertake shifts on Single Point Access Line
• Clinical triage of referrals
• Completing comprehensive assessments of patients’ care needs, planning, implementing and evaluating care delivery to patients with complex specialist palliative care needs.
• Providing specialist advice in the management of complex symptoms direct to patients with complex palliative care needs and their families/carers.
• Liaising with other organisations and stakeholders as part of wider Multi Disciplinary Team to ensure safe, effective delivery of patient care.
• Providing formal training to develop the palliative care skills of multi-disciplinary teams, including pre and post registration students.
• Demonstrates specialist professional practice
