This is an exciting opportunity for a caring, compassionate and motivated individual to work with our Clinical Nurse Specialist and Chemotherapy teams in the Haematology Unit. This post will support the existing navigator in providing support and signposting to patients receiving treatment or monitoring for haematological conditions. This will also support some of the remote monitoring undertaken for this patient group.
The purpose of this role is to complement the existing team to support patients through diagnosis, treatment pathways and follow up to ensure the provision of safe, seamless, appropriate services, thus maximising the health and quality of life of the patient and carers and improving the quality and efficiency of health care delivery.
These is a new role supported by Cancer Alliance monies for 12 months initially but with the hope that the concept of the role can be proved longer term.
Coordinate care with the person with living with cancer so they can access the right help at the right time, providing a seamless experience of care. Check that the person is able to self-manage where appropriate and provides a flexible and personalised response to any changes in need.
Be the single point of contact for all patients on an Oncology pathway of care and to coordinate care and track, feedback of results for non-complex cases, managing patient concerns, facilitate rapid re-entry into the system if required and coordination of any necessary assessments, appointments, investigations and organisation of MDTs, virtual MDTS and outcomes in order to fast track people and provide a seamless patient pathway Early detection of problems/ actions to reduce in-patient admissions.
Signposting to other Macmillan resources and services available to meet their identified needs, liaising with the Haematology cancer team as appropriate.
The successful applicant will become a vital part of the Haematology Cancer Team who are a dynamic, fast paced and patient driven team. You will also be the patients link into the hospital and will connect with all other departments within the Trust as well as other Trusts.
