We are a 20 bedded community hospital delivering high quality care in a rehabilitative setting.
In 2017 we were given the opportunity to take part in an exciting new initiative called Discharge to Assess. Our role is providing rehabilitation and reablement to patients both from the acute and home, to assess their needs and aim to discharge back to their own home within 14 days. This role is Pathway 2.

Main duties of the job
You will work as a member of the multi-disciplinary team and will undertake patient assessment to determine patient need, plan and implement their care with a view to ensuring the highest standard of clinical care and discharge planning to meet the needs of the patient.
You will possess excellent communication skills and a willingness to develop. We are a progressive forward thinking Unit aiming to offer the highest standard of individualised care for our patients.

Job description
Job responsibilities
Manage the nursing team when acting as the most senior nurse on duty.
Assess patients needs and make appropriate plans for care, implement care plans and evaluating effectiveness.
Effectively manage the patients journey through admission treatment and discharge
Contribute to own development and the development of others, working in accordance with the ward philosophy for rehabilitation, and NMC Code.
