You will be expected to develop, plan, coordinate and deliver the service whilst working as part of the organisations multidisciplinary team. The MHP will work as an autonomous practitioner and be responsible for the provision of assessments, interventions and education. They will have responsibility for managing a caseload of patients with a variety of mental health conditions and will be expected to collaborate with other healthcare professionals both internal to, and external from, the organisation. Main duties of the job Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required Work with patients to support shared decision-making about self-management, facilitate onward access to treatment services and provide brief psychological interventions Work closely with other PCN-based roles to help to address the potential range of biopsychosocial needs of patients Complete comprehensive risk assessments informing outcomes and positively managing risk indicators Provide a range of therapeutic interventions. qualification and has a valid registration with the governing body or holds a recognised qualification in counselling or therapy and/or holds a recognised membership of a relevant professional body
