Main Duties of the Job
Main Responsibilities
* Timely processing and signing of repeat prescriptions
* Conducting structured medication reviews and reconciliation
* Managing patients with chronic diseases
* Assessing and treating minor ailments
* Addressing medication-related queries, patient education, and counselling
* Monitoring and optimising patients' medical therapies
* Delivering patient care in line with current legislation and clinical guidelines
* Collaborating closely with the clinical team to provide personalised, high-quality care

Job Responsibilities
Key Responsibilities
* Perform clinical medication reviews for patients with complex or multiple long-term conditions
* Reconcile medicines after hospital or intermediate care discharge, ensuring continuity of care and reducing readmission risks
* Participate in clinical and multidisciplinary team meetings
* Manage repeat prescription requests, resolving issues within scope of practice
* Ensure necessary monitoring and testing is in place for patients on long-term therapies
* Address and treat minor ailments within professional remit
* Implement changes to medicines based on MHRA alerts, product recalls, shortages, and updated guidelines
* Maintain accurate and detailed records using clinical templates and coding standards
* Lead or contribute to audits and quality improvement initiatives
* Provide patients with clear, independent guidance on medications and prescribing changes
* Support the development and review of medicines management and prescribing policies
* Help ensure prescribing practices are aligned with local guidelines and formularies
* Identify and address clinical risk areas at the network level, supporting service improvement
* Monitor prescribing trends and offer evidence-based recommendations to GPs or take action where appropriate
* Maintain and enhance your own clinical competence through continuous professional development
* Assist practices in meeting Care Quality Commission standards for safe and effective care
Person Specification
Experience
* Experience of working in primary care.
Qualifications
* Registered member of the General Pharmaceutical Council (GPhC)
* Demonstrated experience working within a primary care team
* Previous experience in a general practice setting
Skills
* Strong communication skills and a collaborative approach to patient care.
* Manage acute and chronic conditions with confidence and compassion.
