Job overview
Due to an expansion of the community Palliative Care Nurse Specialist team, we have opportunities for exceptional individuals to join our friendly and supportive community service.
This is an exciting time to join the team as we continue to develop collaborative working with other specialist palliative care professionals, Primary Care Teams, and other sectors including hospital providers, private sector, voluntary services, social services, and any other care service providers.
We are looking for experienced, enthusiastic, and highly motivated individuals to take up the posts of Palliative Care CNS. This post is suited to a nurse with significant experience of caring for patients with life-limiting illness who possesses advanced clinical, teaching, and communication skills.
The posts will be based at Florence Nightingale Hospice or Marlow Hospital, and a car driver/owner is essential.
Main duties of the job
* To provide specialist palliative care, advice, and resources to patients with life-limiting illness and their carers, in the most appropriate setting.
* To undertake the comprehensive assessment and management of patients including those with complex presentations.
* Collaborate with other providers of palliative care to work towards providing equity of care across Buckinghamshire.
* To work as an effective team member within the specialist palliative care service, collaborating with other specialist palliative care professionals, Primary Care Teams, and other sectors.
* To provide education, advice, and support to professionals, patients, and all carers working with patients.

Education, Qualification & Training
* Registered nurse with current NMC registration.
* Palliative care degree or working towards.
* Non-medical prescribing qualification or commitment to undertake.
* Evidence of continuing professional development.
* Advanced Communication skills course.
* Teaching and assessing qualification.
Experience
* Experience in a palliative care setting or related specialty.
* Experience of working in multidisciplinary teams and other agencies.
* Experience of working at Band 6 level.
* Experience of teaching, audit, and research.
* Experience of community working.
