Care Home in Reach team, HOURS PER WEEK Wehave an exciting opportunity for a Qualified Nurse to join our well-establishedCare Home In-Reach Team which is a countywide service. The team acceptsreferrals for individuals of any age with a dementia diagnosis/ memory issuesand functional mental health illness over the age of 65. The team providestraining to care home staff. The service is open Monday to Friday 9-5 excludingbank holidays. Flexible working patterns will be considered. The team is based in the grounds of theBelmont Abbey which is peaceful rural location, 2 miles from the main city ofHereford. There is plenty of free parking. Main duties of the job
1. Manage a caseload of patientsto ensure they receive a timely assessment, diagnosis and treatment
2. Monitor treatments
3. Effective and timely dischargeplanning and coordination.
4. Undertake andunderstand physical health observations and take appropriate action wherenecessary.
5. Monitor,evaluate and modify treatment in order to measure progress and ensureeffectiveness of intervention.
6. Apply a highlevel of understanding of the effect of dementia and provide advice onmanagement of the condition, healthy lifestyles and well-being.
7. Carry outcomplex risk assessment.
8. Timely andaccurate reporting and escalation of incidents and safeguarding concerns.
9. Attend regularmultidisciplinary meetings.
10. Participate inundertaking professional cover/duty.
