The successful candidate will be experienced in treating a diverse range of patients in a primary care setting.

Main duties of the job
The post-holder will work with reference to practice agreed protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
We are looking for a practice nurse with qualifications in managing Long Term Conditions such as Asthma, COPD, Diabetes and Chronic Heart Disease.
Support the practice Infection control lead in implementation of standards and provide specialist infection control advice to all staff and clinicians.

We are a small friendly team supported by 5 GP Partners. Our patient list size is around 8000 with a diverse population. We are a GP training and teaching practice and have a strong educational culture.
We are part of Lower Valley PCN and have many additional services such as physio, podiatry etc that is offered to all patients of the PCN.

Job responsibilities
Clinical Responsibilities:
* Identify, assess and diagnose patients with complex health needs across the practice population in accordance with practice policy & protocols.
* Work within the nursing team to deliver a high standard of patient care using specialist practice skills.
* Promote health, participate in the developing programmes to improve health and well-being and meet the specific needs of communities, groups and individuals.
* Work with colleagues in the implementation and evaluation of evidence-based practice to improve the services offered by the nursing team.
* Responsible for delivery of general practice nursing to whole practice population working within own profession boundary.
* Liaise and maintain good working relationships with all members of the practice.
* Work within the NMC code of professional standards.
* Promote and maintain Infection prevention and control- Support the practice lead on IPC standards throughout the practice.

* Nurse Led Chronic disease management.
* Diagnosis, monitoring and development of individual management plans.
* Health promotion and well being.
* Cytology and sexual health screening.
* Wound care.

Qualifications
* Registered General Adult Nurse.
* A commitment to professional development evidence of educational qualifications.
* Full UK Driving License.
* Holding or working towards qualifications in Long Term Conditions management such as Asthma, COPD, Diabetes & CHD.
* Cytology.
* Immunisations Adult & Child.
Experience
* Experience in primary care nursing.
