Our CCN teams provide specialist paediatric nursing care to babies, children, young people (BCYP) and their families in a variety of community settings. This new clinical Nursing leadership role will enable a specialist CCN to provide care to patients receiving oncology treatment and nursing care provided by the CCN teams across Portsmouth and Southampton. They will work closely with the Paediatric oncology primary treatment centre in Southampton Children's Hospital as well as the shared care unit in Portsmouth Hospitals to ensure seamless care between hospitals and the community for the paediatric oncology population. Main duties of the job Manage a caseload of babies, children and young people (BCYP) across Portsmouth and Southampton CCN teams, ensuring high quality, safe and compassionate community services. Be a direct point of contact and resource for BCYP at diagnosis and throughout their cancer trajectory. Act as a clinical lead, demonstrating a high level of expertise and autonomy within the CCN service, providing specialist advice, education and support to patients and professionals. Demonstrate a high level of clinical skills and expertise within paediatric oncology, palliative and end of life care services, leading and participating in a 24 hour end of life on call service as required. Lead on clinical education of specialist oncology nursing in community settings. Collaborate with the CCN team, PTC and POSCU to deliver seamless care for children with an oncology diagnosis. Benchmark and maintain a high standard quality of patient care. Have a focus on leading improvements in the quality of care for BCYP and families living with an oncology diagnosis in the community. Work across professional, departmental and Trust boundaries as required to develop and support patient-centred care.
