You will be the nurse in charge when on shift ensuring through good communication and leadership your team provide our residents optimum care. Carrying out medication rounds using our electronic systems, managing stock, carrying out CD checks and to escalate any medication changes, concerns accordingly to pharmacy teams / home management team. Daily documentation of actions and communications on our e-plan, managing and updating care plans for residents who you will be assigned as named nurse. To be aware of each service user's needs and knowing when to communicate with outside agencies and multi-disciplinary teams to provide the best outcomes for our residents. Knowledge of recognising and managing any decline in health physical or mental. Mandatory training to remain in-date. Person Specification Qualifications Essential You must be a Registered General or Mental Health Nurse with a valid NMC PIN, with no restrictions to work. Desirable NMC Registered Skills Essential The ability to assess care needs and produce care plans for residents that reflect their needs and wishes is essential. Many of our residents have advanced dementia and mental health diagnoses, which can make it difficult for them to express their wishes at times, we are looking for nurses who are able to help our residents to maintain a good and satisfying quality of life and be cared for in the way they would like and which also meets their needs. Desirable Basic IT skills are required for this job, as we now use electronic care planning and medication systems. Training will be given in our systems.
