We are looking for enthusiastic, reliable and approachable individuals to take on the challenge of being the newest member of Birmingham Community Healthcare Charity Team.
The Direct Access to Wellbeing service (DAWS Team) is a brand-new project team that is part of the wider BCHC Charity. The post will involve supporting individuals while in the care of BCHC, following discharge to home, at community events and following referrals from our partners, to offer advice and guidance, along with practical measures to support people to live better for longer and to be well and warm at home.

Key Responsibilities:
1. Deliver early intervention signposting and support to patients, their families and the wider community.
2. Have an awareness of BCHC health services and the wider NHS Support available, allowing you to signpost individuals as required.
3. Offer health and wellbeing guidance, early intervention health messages such as healthy eating, oral health and falls prevention.
4. Provide energy saving tips, signpost individuals living in fuel poverty, and offer a simple benefit check and referral for further support.
5. Demonstrate a high level of communication and interpersonal skills in dealing with a wide range of NHS staff and external agencies.
6. Engage with individuals face-to-face and via telephone to offer signposting, support, equipment, funding, and additional items as required.
7. Link with the wider BCHC services, NHS services and voluntary sector to ensure individuals are aware of available resources.
8. Manage project engagement activities within a certain area of the BCHC profile ensuring project objectives are delivered and deadlines achieved.
9. Understand the needs of the beneficiaries and collect necessary information to offer appropriate support including signposting and referrals.
10. Manage feedback and complaints from patients or individuals and seek to resolve issues proactively.
11. Communicate effectively, appropriately and compassionately with patients, carers and families.
12. Produce information in accessible forms, considering the diverse needs of our patients.
13. Engage with individuals to give specific advice related to health services, energy efficiency, and initial benefits screening.
14. Support stock levels ensuring items are regularly ordered and raise requisitions as specified.
15. Support in the scoping and delivery of events, ensuring the project has a visible presence.
16. Record, analyse and monitor evaluations following attendance at events or after contact.
17. Maintain confidentiality in all aspects of duty including patient and carer data.
18. Support junior staff with training needs and provide direction and supervision.
19. Delegate tasks appropriately to junior staff as needed.
20. Provide senior staff with regular updates regarding issues affecting individual and team capacity.
21. Coordinate, monitor and solve scheduling of project activities.
22. Follow agreed branding and formatting of materials and standards within the BCHC Charity.
23. Support monitoring of the team generic email inbox, responding appropriately and timely.
24. Provide support to the wider BCHC Charity as required.
25. Engage in high-level administration duties such as answering phone calls and emails.
26. Promote the wider charity objectives and activities to increase reach and impact.
27. Attend and participate in team meetings and other ad hoc meetings when necessary.
