Discharge to Assess Coordinator
Band 6
Job overview
Are you looking to work in a dynamic, flexible and forward-thinking team – ready to adapt and accommodate every eventuality to meet patients' needs safely? If so, Care Coordination is the team for you.
We ensure variety in our work and opportunities for professional development – our drive and determination to meet the needs of our patients meant that we were HSJ award finalists in September 2022.
Main duties of the job
The Discharge to Assess (D2A) Team are a nurse-led team coordinating discharge of patients with complex health needs. The team receives referrals from the acute setting and works collaboratively with Social Care and community therapy services to identify an appropriate discharge pathway.
Taking a ‘Home First’ approach we help provide patients with reablement at home or short-term care and ‘stepdown’ beds to bridge the gap between hospital and home to facilitate assessments of ongoing health and social care needs. Wherever possible, patients should be supported to return to their home for assessment.
We are looking for an enthusiastic, pro-active band 6 nurse with a keen interest in hospital discharge to join our team. You will need excellent communication and interpersonal skills working with the patient, families, and the multi-disciplinary team to coordinate safe, timely discharges. In addition to the above, as a Junior Sister within the Care Coordination Team you will be required to provide managerial leadership support to the team.
If you are a key team player and committed to promoting a seamless service, we would really like to hear from you.
Working for our organisation
To support you in achieving the above we will endeavor to provide you with:
* A robust 4-week induction plan to gain knowledge of the service and the other services that we work with in both the community and acute setting.
* An allocated buddy to support you throughout your induction and beyond.
* The opportunity to work within a team consisting of nursing staff, therapists, pharmacists, MDT coordinators and administrative staff.
* Use of the most up to date IT applications to support remote patient triage and assessment.
* The opportunity to gain new skills and ensure the skills you have are kept up to date.
* Development opportunities including training offered by the trust and the opportunity to complete external University modules.
* A team that will support your creativity – offering you leadership exposure and supporting you to lead on service improvement and ensuring innovative practice with support of the senior leadership team.
* Access to health and wellbeing services, and access to local Professional Nurse Advocate support.
* A flexible team that will support your needs to achieve an effective work-life balance.
