An exciting opportunity has arisen for a full time Band 6 Primary Care Mental Health Practitioner to join our Solihull Neighbourhood Mental Health Team.
Within BSMHFT, our community mental health services are well-established and as part of the BSOL community transformation programme, we are looking to increase our primary care teams with primary care practitioner roles. This is an exciting opportunity to be at the heart of national change and developments in the delivery of community mental health care. The development of our Primary Care mental health team is an essential service which we are in the process of evolving in line with the NHS Long Term Plan.
We are looking for enthusiastic and dynamic individuals who are able to work within a multi-disciplinary team, working alongside GPs and other primary care professionals who strive to achieve the best outcomes for individuals within the local community.
You will be responsible for providing mental health triage, assessment, and outcomes-based intervention to meet service users' needs within the Primary Care Network. You will be responsible for identifying need and the best course of advice, support, or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.

2. Promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
