Roles and Responsibilities will include:
Working with young people who are Looked After and Care Leavers, aged 16 to 25 years.
Working with unaccompanied Asylum Seekers.
Providing a holistic service to young people, assessing needs to provide the best possible support, advice, guidance, and planning.
Working in partnership with key agencies and support networks.
Completing Pathway Plans that are robust, reflective, and based upon individual needs.
Supporting young people in making wise choices, staying safe, and reaching their full potential.
Supporting the housing needs of young people as they transition into adulthood.
Supporting young people in meeting their educational potential and engagement in education, employment, and training.
Supporting young people to meet their health needs.
Ensuring that young people's wishes and feelings are at the centre of all practice and that their views help shape interventions and practice, which is recorded within their Pathway Plan.
Ensuring accurate records are maintained, reflecting decision-making.
Supporting the team and young people through the duty service and social groups.
Have a genuine interest in working with young people.
Demonstrate an enthusiastic and dynamic approach to their work.
Have an Enhanced DBS.
Hold a minimum NVQ Level 4 or equivalent, or be working towards NVQ Level 4.
Must have a full driving licence and use of a car with business insurance
