Job overview
The post holder will primarily work as part of an established multidisciplinary team within the Community Hospital, assisting the professional multidisciplinary team to provide and deliver quality patient care.
Main duties of the job
We are looking to appoint a Band 2 Health Care Support Worker who is caring, enthusiastic, and motivated to join our established integrated intermediate care team, primarily based at Castleberg Hospital in Giggleswick. The Intermediate Care Teams have undergone recent developments and have expanded to include Castleberg Hospital.
The service at Castleberg provides bed-based intermediate care to support people who are in an acute but stable condition and also provides palliative care to people during their last days of life if they cannot be cared for safely in their own home.
The Intermediate Care Team includes Physiotherapists, Occupational therapists, Mental Health Nurses, Advanced Nurse Practitioners, Staff Nurses, and Community Support Workers. It is important to be able to work within a multidisciplinary team as well as independently. We utilise the Calderdale competency framework for skill sharing.
Applicants must be prepared to work shifts covering 24/7 and have access to own transport, with car driver and access to vehicle essential (Car leasing schemes available). The successful candidate may be required to rotate across Intermediate Care services, providing a great development opportunity to develop skills and increase knowledge across a range of community settings.

Qualifications & Training
* NVQ level 2 or equivalent in Healthcare/Health and Social Care
* Commitment to work towards Health and Social Care Diploma Level 3
* Evidence of ongoing training
Experience
* Experience of working in a care environment
* Experience of teamwork
* Some recent experience in care settings
