An exciting opportunity is now available at the Provider Collaborative ACAH community Enhanced Falls service for an enthusiastic, forward-thinking Occupational Therapist to work in a senior decision-making role within ACAH 2hr UCR Urgent Community Response Service.
This post sits with the UHNM ACAH team but works with the UHNM/MPFT therapies team.
The Enhanced Falls service Occupational Therapist will work alongside an ACAH Acute Care Practitioner responding to falls and sick patients in the community, providing urgent holistic assessment of mostly unwell elderly patients at home.
The successful candidate will have previous NHS experience working in emergency portals/community services or have a range of transferable skills including assessing falls patients. The role will have flexible 7-day working. Falls service operational service times would currently be from 0900 to 2000hrs.
We are keen to meet dynamic, enthusiastic, and highly motivated Occupational Therapists interested in working in our supportive admission avoidance team providing the highest standards of care to keep our frail patients safe in their own homes.
Our focus is the reduction of ED attendances and safe admission avoidance, acute/subacute interventions to improve functional outcomes & specialist falls assessment and prevention.
Applicants will need to hold a full driving licence as this is required for the role. A vehicle is provided to enable all necessary equipment to be carried by the team.
Main duties of the job
This role works autonomously as a specialised practitioner managing the clinical delivery of care within Acute Care at Home. The Advanced Therapists lead and organise the resources available to meet service standards and workload demands within Acute Care at Home Enhanced Falls.
This post may encounter patients that include the critically ill, unconscious, unstable, neurological, acute and chronic respiratory, multi-pathology/complex patients, cognitively and perceptually impaired.
