Join our dynamic Community Speech and Language Therapy Team as a Band 7 Highly Specialist Speech and Language Therapist and team lead across Newport and Blaenau Gwent. In this role, you will have the unique opportunity to work directly in people's homes, helping them achieve meaningful and personalized goals. You will be an integral part of a supportive and collaborative team, working alongside our partners in primary care and community resource teams to deliver holistic care.
Main duties of the job
We are seeking a compassionate professional who thrives in a team environment and is passionate about delivering high-quality care. Dysphagia competence is essential for this role. Your expertise will be crucial in developing and implementing individualized care plans for dysphagia and communication, within a place-based care model. You will work with clients with a range of disorders including progressive neurology, dementia, and frailty, focusing on supporting individuals to remain well at home, avoid hospital admissions, and facilitate discharge into the community from community hospital sites. We are actively looking to secure investment to expand our reach, and therefore there is potential for additional hours beyond the end of the contract date. Full and part-time hours will be considered.
This post is fixed term for 12 months due to the needs of the service. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
