JOB DESCRIPTION
Job title: Carers Community Navigator (substance misuse)
Pay Scale: £25,000
Hours: 37.5 hours per week
Holidays: 25 days plus Bank Holidays
Reporting to: Operations Director
Aims of the post -
To support unpaid carers and their families in Blackpool with a focus on the impact of problematic substance (including alcohol) misuse. Work together with families where substance misuse is the primary issue relating to the provision of a caring role.
This post is subject to DBS clearance at enhanced level
JOB PURPOSE
The post holder will provide direct support to carers and their families with a focus on substance (including alcohol) misuse issues and the impact of this on family life.
The key objectives of the role are to:
Work within the community to provide timely information and support to carers and their families.
Work with carers and their loved ones to understand and access appropriate treatment, support and recovery services to address problematic substance misuse.
Work with our Respite Project to support carers to access meaningful time out from their caring roles to take part in activities and peer support to develop friendships, positive networks and skills.
Support carers in managing their caring role and maintaining their own mental health and well-being through pro-active practical and emotional support.
Promote awareness and identification of carers across professional services and
within the community through information events and training facilitation.
Provide effective, outcome focused reports in line with funder requirements to demonstrate that identified objectives are sufficiently evidenced.
KEY TASKS & RESPONSIBILITIES
To be based at Blackpool Carers working within the community, including substance misuse treatment services, to identify carers and their families who require support.
To work collaboratively with colleagues within Blackpool Carers and wider agencies to develop and embed person-centred pathways for carers and their families.
Assessment of needs within families to identify appropriate levels of support, required services and development of short-term support plans, enabling carers and their families to make informed choices to protect their physical and emotional health and wellbeing.
To provide verbal and written reports as required, including statistical data and qualitative feedback to demonstrate that the aims and objectives of the project are being met.
To maintain an up-to-date knowledge of developments in local and national strategy and apply this knowledge in supporting carers and families.
To provide intensive, one to one emotional support where appropriate.
To maintain an effective database and accurate records for carers and information relating to them and respond to reasonable requests for statistical information from funders, in full compliance with data protection legislation.
To undertake assessments with carers and families using Blackpool Carers assessment tools and refer to statutory or other voluntary organisations, including Blackpool Council for a Carers Assessment where appropriate.
To carry out effective consultations with carers and the people they care for ensuring that all services are developed in line with carers’ needs. Promote their active involvement and participation in developing local services.
To work in partnership with existing providers to ensure that carers and the person being cared for has access to appropriate support or representation
To work effectively with Blackpool Carers colleagues and regarding external communications
To attend training courses and events as required
To contribute to the co-ordination, training and support of volunteers and students within Blackpool Carers. Work closely with volunteers and students
To work across 7 days, including evenings, weekends and school holidays
OTHER DUTIES

To work positively and flexibly as part of the team to ensure the smooth day-to-day running of services for carers
To attend and participate in regular internal staff meetings
To undertake other tasks as required. To work under the direction of the Operations
Director, adhering to all policies and procedures and represent carers at appropriate meetings
Duties of all staff are periodically reviewed and may alter in line with service requirements and developments in consultation with staff.
