Job summary We are looking for a dynamic, organised and motivated individual who is keen to support medicines optimisation and patient education in the management of renal and transplant patients. You will be joining the UHL Leicester Kidney pharmacy team, which is committed to innovation and improvement of the care we provide to benefit kidney patients. Some of our highly specialised clinical pharmacy services include: immunosuppression trials and participation on ward rounds and MPT meetings, writing peri-natal medicines care plans and producing novel education materials for patients in video format. Not only are we are an integral part of the wider kidney multi-professional team in UHL but we have been working closely with primary care and leading the multi-award-winning LUCID programme. Main duties of the job The post holder will work closely with medical staff, nursing staff, allied health care professionals and management teams within the Renal, Respiratory and Cardiovascular CMG to provide and support safe, high quality and patient centred care services to renal and transplant patients. The post holder will need to undertake audit, service improvement projects and review clinical practices to affect positive change in medicines management. The post holder will be required to attend clinical meetings when appropriate, maintain and develop formal links with clinical teams, support junior pharmacy staff and provide relevant drug information as requested to healthcare professionals. The post holder will monitor drug expenditure within the directorate, advise on the most effective treatment options, and through production of financial reports to the RRCV CMG lead and the relevant teams. The role will include line management responsibilities, training and development of less experienced team members including trainee pharmacists, and band 6 and 7 pharmacists in renal and transplant. As part of the job, the post holder will be expected to contribute to core pharmacy services including weekends, bank holidays early evening shifts and on-call on a rotational basis.
