Job Summary
Our work in Primary Care Networks (PCN) is at the very heart of Stockport PCN Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time".
We are looking for a flexible, innovative practitioner with a variety of clinical skills. The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers.
Due to the nature of the role, it is essential that applicants are able to travel independently across Stockport and have access to a suitable vehicle for business purposes. Where possible, adjustments can be considered in accordance with the Equality Act.
Main duties of the job
Bramhall PCN is in the position to offer exciting new opportunities for a Band 3 Support Worker. We are looking for an enthusiastic individual who would work with the Community Nurses and Adults Social care staff to provide a first-class service to the residents of Stockport. Line management will be provided by Stockport foundation trust, under NHS terms and conditions. On appointment, successful candidates will be expected to complete the Care Certificate.
1. To work within a multidisciplinary team delivering social and nursing care (appropriate to the post) to clients in their own home. Care delegated by an appropriate practitioner utilising personalised care plans as appropriate.
2. Providing patients with support in administering medications, eye drops, wound care and supporting your colleagues.
3. Working 34hrs/week between 8am-6pm.

1. To assist with general wellbeing; creating a supportive and re-enabling environment promoting maximum independence.
2. The scope of the role will focus on supporting admission avoidance, early supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and Palliative patients symptom control.
3. If candidates who are appointed do not hold the Care Certificate, an opportunity to complete will be available.
4. The post holder MUST hold a driving licence with access to a car to fulfil this role.
