Job summary We are looking for an experienced Dietitian to cover a Dietetic service to adult patients across the community services in the Coastal Locality (Teignmouth and Dawlish). The case load will consist of a wide range of clinical conditions including frailty and older people with multiple long-term, complex conditions. The Dietitian will assess and treat patients in community settings including care homes and patients own homes, with the main focus on malnutrition. You will work as an autonomous practitioner and alongside community multidisciplinary teams including Intermediate Care, Community Nursing, Health and Social Care teams, and Care Homes. The Dietitian will be the nutrition expert within those teams, being a highly skilled Dietitian with a dynamic, self-motivated and innovative approach to your work. You will be passionate about patient care, with excellent communication and engagement skills, team working and strong time management. The job is based at a number of community locations including Teignmouth, Torquay (Community Dietetics base), as well as working from home. You must be able to travel independently and have access to a car on a daily basis. Working pattern to include Wednesday (for weekly triage/clinical supervision with the Community Dietetic team). The post is a 0.5 WTE (18.75hours/week) fixed term contract for 12 months maternity cover, with possible extension/permanent hours depending on movement within the team. Main duties of the job You will provide a Dietetic service to adult patients across the community services in Torbay and South Devon, with the main focus on malnutrition and oral nutritional support. Dietetic assessment and management of patients living in the community (own homes and care homes). You will work alongside and support health and social care professionals within community MDTs. To attend regular team meetings to discuss the nutritional care of patients, as part of their multi-disciplinary care.
