An exciting opportunity has arisen for a Advanced Clinical Practitioner to join the Acute Frailty team. Applicants must hold an MSc in Advanced Practice (or equivalent award), or near completion of their training. Evidence of advanced practice competencies, or undertaking. The post holder will be registered as an Independent Prescriber. Interested applicants must have significant experience at a senior level working within acute frailty or an acute clinical setting and have experience to support the ability to see and treat patient autonomously within an acute / urgent care environment. Trainee Advanced Clinical Practitioners with Acute Frailty or Urgent Care experience, who are near to qualifying within the next 6 months are encouraged to apply for this post. The post holder will be based at Airedale NHS Foundation Trust with cross cover within Urgent Care and the Elderly Medicine directorates. The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients. The ACP is accountable for the total episode of care for patients with undiagnosed and undifferentiated conditions. The post holder will be a qualified Advanced Clinical Practitioner or near completion of their training. Have evidence of advanced practice competencies. The post holder will act as a role model, delivering advanced practice and promoting clinical effectiveness in care. He/she will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients. The job holder will work as an autonomous practitioner within a defined clinical area, delivering all aspects of management and care to a wide range of patients with varying needs. They will work closely with all members of the multidisciplinary team and will play an integral role in the operational/strategic development of the service.
