An exciting opportunity has arisen to join our vibrant Adult Community Nursing Service. Homerton Healthcare NHS Foundation Trust is an integrated community and acute trust providing high quality service to residents of City and Hackney borough.
We are seeking an enthusiastic Band 5 nurse to fill a full-time permanent post. The Adult Community Nursing Service is divided into 8 neighbourhood teams. Successful candidates will be asked to work within one of the neighbourhood teams.

Main duties of the job
1. The post holder will practice within a multidisciplinary team and provide care to patients within their own home and community settings.
2. Assessing, planning, implementing, and evaluating care needs under the supervision of Clinical Nurse Specialists, Practitioners, and District Nurses.
3. Responsible for delivering a high standard of holistic nursing care while promoting and maintaining health education and independence.
4. Take responsibility for their own actions when delivering clinical care; maintaining competency and skill in line with role requirements.
