The Mental Health Advanced Nurse Practitioner will beresponsible for the assessment of health and clinical needs of the patient. Toundertake consultations with patients as an autonomous practitioner usingadvanced clinical practice skills, own clinical judgement to diagnose, treat, refer,or discharge patients.
The post is a casual one whereby shifts are requested and allocated via an app-based rota system. We are currently looking for an Advanced Mental Health Nurse Practitioner to join the 7-Day GP Service team who are flexible to work from 5pm in the evenings between Monday to Friday and throughout the weekend during the daytime.

Main duties of the job
The main duties include but are not limited to;
1. Undertake consultations with patients taking acomprehensive patient history, carry out physical examination use their expertknowledge and clinical judgement to identify the potential diagnosis
2. Exercise a higher degree of personal andprofessional autonomy using advanced clinical practice skills to includeclinical interventions
3. Refer patients for investigations whennecessary/appropriate
4. Make a final diagnosis, decide on and carry outtreatment, including the prescribing of medicine or referral to primary orsecondary care specialist services within local agreed pathways and protocolsas appropriate
5. Ensure that all treatments and care provided arebased on best practice/evidence-based care
6. Plan and provide skilled competent care to meetthe patients health and social care needs, involving other members of thehealthcare team as appropriate
7. Work with multi-disciplinary and multi-agencyteams in order to ensure patient needs are met
8. Asses and evaluate, with the patient, theeffectiveness of the treatment and care provided and make changes as needed
9. Use independent/supplementary prescribing skillsfor benefits of patient care
10. Work independently and as part of the 7-Day GP Serviceteam

23. Identification of emergency care needs requiringdespatch of a 999 ambulance
24. Safeguarding children and protection ofvulnerable adult issues which require referral on to other agencies in linewith local policies
25. Work independently and at the same timecontribute to the work of the whole team to ensure the delivery of aconsistently high level of service, this will include:
26. Taking responsibility and being accountable formanaging own workload, risk assessment and management, acting withinprofessional codes and appropriately to the clinical team leader for advice andsupport
27. Undertaking other duties when required, such asaudits or administrative tasks
28. Identifying issues which may negatively impactupon service delivery and reporting these on to the clinical supportmanager/clinical manager operational team leader/shift manager in a timelyfashion
29. Acting as a clinical resource for non-clinicalstaff who may require advice and support
30. Maintain contemporaneous records of theconsultation and action using clinical software and where necessary writtenrecords, in line with NMC guidance and local policies, guidelines andprocedures
31. Remain focused on the delivery of an excellentservice within an unpredictable, diverse, and challenging workload
32. Work remotely using EMIS Web following theWorking At Home Policy ensuring records and patient information is protectedfollowing GDPR.
