Responsibilities:
– Assess clients’ needs, capabilities, and goals, then develop personalized reablement plans focused on improving independence.
– Provide direct support and coaching to clients, teaching new skills and promoting self-care to help achieve reablement goals.
– Monitor progress, adjust plans as needed, and collaborate with healthcare professionals and community services to ensure holistic support and advocacy for clients.
What we are looking for in a Social Worker:
4. Social Work England registration
5. Eligible to work in the UK
6. Hold a full UK licence
7. Post-qualified experience
8. Degree level or equivalent in Social Work
