Job summary We are recruiting for a Rehabilitation Assistant to join our team on the Isle of Wight. Our team are looking for someone to work as part of the Community Rehabilitation Team to provide efficient and effective multi-disciplinary care as delegated by a Registered Nurse or Therapist within the Community Rehabilitation Team. The Community Service delivers a 7 day Rehabilitation service across NHS commissioned Nursing home beds and patient's own homes. Main duties of the job The successful candidate will deliver a range of delegated nursing and therapeutic interventions which focus on the direct rehabilitation needs of the patient under the care of the Community Rehabilitation Team. Working within the boundaries set out by the Registered members of the Community Rehabilitation team (i.e. patients that have already had an initial assessment by the relevant staff members), the scope of the role enables the post holder to make accurate observations of a patient's ability, and feed this information back to the registered member of staff, in the following circumstances: Washing and dressing Meal preparation Progression of patients with walking aids (crutches to walking stick for example) Transferring from chair to bed etc and monitoring progress with this Progression of patients with stair practice Visiting patients at home and fitting of certain prescribed equipment Bathing assessments post discharge of patients from Community Rehabilitation provider beds About us Hampshire and Isle of Wight Healthcare NHS Foundation Trust is a newly established organisation, combining the expertise and services of multiple NHS providers to deliver high-quality healthcare across Hampshire and the Isle of Wight.
