This includes reviewing drug charts for safety and efficiency, drug history taking and medicines reconciliation, medicine supply, patient medication consultations, medicines optimisation services, discharge medication service (DMS) and any other activities associated with medicines management. To participate in departmental and regional education and training programmes. In particular, to undertake study towards obtaining the Certificate/Diploma in General Pharmacy Practice. To provide pharmaceutical advice on both supply and clinical issues to other healthcare professionals as requested. To provide medication related advice to patients, focussing on medicines stopped/started or changed in hospital and side effects profiles enabling them to obtain the maximum benefit from their medicines. To assist in the education and training of pre-registration pharmacists and other staff in training grades. To supervise the daily activities of junior pharmacy technicians and pharmacy assistants.
