An interesting opportunity has arisen for an Adult Care Social Worker to join the Community Hospital (CH) and Community Support Beds (CSB) Discharge Team primarily to help support the flow of people aged over 18 from Community Hospitals. As the team also supports discharges from Community Support Beds within DCC care homes, you will need to be flexible to maintain cover during planned and unplanned absences of other team members. The post is a part-time permanent post (working days negotiable) and will initially focus primarily on discharges from Cavendish Hospital in Buxton, although you will need to be prepared to travel to the Stamford Unit in Ashton-under-Lyne and other Community Hospitals or CSB units throughout the county of Derbyshire. Mileage and travel time will be paid in accordance with the current travel policy and the post will be based at the Buxton Area Office.
The post holder will be a key player in supporting health and care systems during a period of increasing pressure on hospital beds. The postholder will be required to form close relationships with hospital staff as key members of the Multi-Disciplinary Teams, working within the hospitals on some days.
Experience of assessment and reviewing are desirable together with a commitment to person-centred practice, taking a home first approach in line with the Best Lives initiative. You will need good organisational and time management skills together with a knowledge of local resources, to achieve timely, sustainable discharges and positive outcomes for the people who are being discharged from Community Hospitals (and Community Support Beds). For less experienced applicants, transferable skills will be considered, providing that you have the willingness and potential to learn. You will also require excellent communication and inter-personal skills to both support the people and their families, whilst enabling you to liaise and maintain positive relationships with hospital staff, care home staff, and workers from other disciplines.
