Complex Care Nurse Assessor - Huntington's Team
To undertake comprehensive nursing assessments and reviews to determine the complex nursing needs for individuals with Huntington's disease.

We currently have a vacancy for a Band 6 Nurse Assessor in our Multi-Disciplinary Community HD Team. The Team provides specialist assessments and interventions to clients and family members affected by HD across the ABUHB catchment area. Working within the community, primarily seeing people in their own homes, working with clients and their families, but also working with clients and staff in Nursing Homes, Group Homes, and inpatient facilities as required.
This is a community post, so the ability to travel between sites in a timely manner is essential.

Job Responsibilities
The successful applicants should have:
* Recent NHS or Care Home experience within the last 12 months is essential.
* A proven track record of being reliable and dependable.
* A flexible approach to work would be a great advantage.

Person Specification
Qualifications
* 1st level Registered Nurse (RGN / RMN / RNLD) registered with NMC
* Degree level education
* Postgraduate specialist training in Community care/nursing or relevant area of nursing or equivalent
* Adult Safeguarding level 1 & 2
Experience
* Relevant post-basic experience at band 5 or above.
* Experience of undertaking needs-based assessments and care plans
* Undertaking capacity assessments, gaining formal consent and the application of DoLS and the Mental Health Act.
Knowledge
* Dealing with cases with complex presentations
* Knowledge of Continuing Care legislation and policy
* Evidence of Communication training
