Job Title: Diabetes Nurse/ Practice Nurse
Salary: Dependent on experience
Hours: Part-time, 20 hours per week
An exciting opportunity has arisen in Crossgates Medical Practice for a qualified and experienced Diabetes Nurse to join our friendly practice team. You will provide specialised Diabetes management, treatment and education to patients in a Primary Care setting, and act as a resource for expert advice for the wider clinical team.
About the role:
As a Diabetes Nurse, you will play a key role in managing and supporting patients with diabetes, working independently as well as part of a team to deliver a high standard of patient care. Your role will involve providing comprehensive diabetes management, including education, monitoring and treatment to patients. You will also support patients with prevention strategies, lifestyle changes and initiate treatment for newly diagnosed patients.
Key responsibilities:
· Provide specialist nursing care for patients with diabetes
· Educate patients on the management of their diabetes, lifestyle and prevention strategies
· Carry out regular monitoring of the patient’s diabetes condition, including making changes to their medication and maintaining accurate patient records
· Work closely with the GPs, ANP, Practice Nurses and other team members to provide integrated care
Essential
· NMC Registered
· Non-medical prescriber
· At least 2 years post-registration experience in diabetes care, either in practice, community or hospital setting
· Comprehensive knowledge and experience in the management of diabetic patients, and the initiation of treatment in newly diagnosed patients
· Evidence of Continuing Professional Development
· Competent in the use of IT
Desirable
· Diabetic diploma
· Treatment room experience – basic wound care and compression
· Chronic disease management
· Cervical screening
· Previous experience in a primary care or doctor’s surgery environment
About us
We are a small, close knit GP surgery based in Crossgates, Fife with a practice population of approximately 4,500 patients. Our practice team is comprised of 2 GP Partners, an ANP, a Practice Nurse, Practice Manager and 7 admin staff. We also have a number of attached staff supporting us as part of the wider Multi-Disciplinary Team, including CTAC Phlebotomy, Treatment room and a Mental Health Nurse.
