Job Title: Social Worker – Hospital Team
Location : Cheshire West And Chester Council
Rate : £35 per hour
An exciting job role has recently become available in Cheshire. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
In this role, you will assess individuals’ and families’ needs, including identifying risks, determining intervention methods, and advising on suitable services and resources. You will collaborate and negotiate with professionals, statutory bodies, and voluntary agencies to ensure optimal services for clients. You will manage and prioritize an allocated caseload, adhering to county policies and statutory requirements. Additionally, you will maintain accurate records of all work undertaken and follow necessary administrative procedures. You will participate in supervision, staff meetings, and training, and contribute to service development by sharing knowledge and supervising students where appropriate.

Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
